Listing Specs:  Advertiser authorizes TDI to furnish space for Listings submitted by Advertiser 
in the Blue Book.  Advertiser, or its agents, agrees to submit Listings to TDI on paper (in addition to 
e-mail if so desired) by November 1st in order to have them listed in the following year’s Blue Book.  
Furthermore, Advertiser agrees that Advertiser is responsible to ensure completeness and 
accurateness of Listings submitted to TDI.  TDI reserves the right to reject any Listings submissions 
unless they are printed and/or faxed clearly and are acceptable for inclusion in the Blue Book.

Rates:  The first Listing in each state will be charged $50.00 (for non-profit organizations or 
businesses with 15 or fewer employees) with $20.00 for each additional listing in the same state as 
the first listing.  The District of Columbia, Puerto Rico, the U.S. Virgin Islands, other U.S. possessions
and territories, and foreign countries each are also considered as "states" for this purpose.Discounted Rates for TDI’s Blue Book Advertisers:  By also having, at least, one 
advertisement in the same publication of the Blue Book, Advertiser will pay only $15.00 (instead of
$20.00 as explained in the above paragraph) for each additional listing in each state.

     If your organization/business has a 8xx toll-free number or a website, you can request to have it
     listed in reverse type in order to make it stand out in the list in the Blue Book:

Revisions:  No additional charges for revisions and deletions of pre-paid Listings from the previous
                          year’s Blue Book.

Cost:  TDI agrees to publish Advertiser’s Listings, as set forth in the above paragraph, for $________,
              hereinafter referred to as the Sum.

Payment:  The said Sum is due and payable by check, money order or credit card, made payable to 
TDI, upon submission of this Agreement to: TDI, 8630 Fenton Street, Suite 604, Silver Spring, Maryland 
20910.  Exception to this rule is made only by submitting for approval the deferred billing option outlined
below.  TDI will, upon receipt of the correct remittance, or approval of deferred billing, sign this 
Agreement and return a copy to the Advertiser. In the event full payment of the said Sum is not received 
by the deadline for inclusion for publication, the Listings will be published only if the request for deferred
payment is approved.

Deferred Payment:  An Advertiser requesting deferred payment of the cost of the Listings, which
may be granted ONLY BY special policy or the requirements of State law, shall do so by signing the 
“deferred payment  requested” line below and returning this Agreement to TDI.  If it grants such request,
TDI will so acknowledge by returning a signed copy of this Agreement to the Advertiser. Deferred 
payment is due in full within thirty (30) days after receipt of the advertisement publication by the 
Advertiser.  TDI reserves the right to charge interest at a rate of 1.5% per month on deferred accounts 
over 30 days outstanding.

Disclaimer and Liability of TDI:  TDI assumes no liability for errors in the Listings caused by 
the Advertiser or its Agents, and TDI is not to be held responsible for any errors and/or omissions that 
might arise.  Advertisers assume full liability for the contents of their advertised listings and any claims
arising from them and will indemnify and hold harmless TDI for any liability arising therefrom.   TDI
reserves the right to edit and position Listings according to the space requirements  for the size of the 
Listings purchased by Advertiser. TDI also reserves the right to reject any Listing.  TDI does not endorse
any product or service listed.  This disclaimer applies to the traditional printing  of the Blue Book as well 
as to any other format, electronic or otherwise. In the event the Advertiser’s  Listings are not published, 
or an error occurs in the Listing though no fault of the Advertiser or its  Agents, the liability of TDI shall 
be limited to a refund of the Sum to the extent paid. The Advertiser has read and understands the terms and conditions of this Agreement. The Listings will be ( circle one)    mailed  /  e-mailed  /  faxed   to TDI  no later than the first of
